Designated Visitor Application
To comply with the Public Health Officer’s order, and Assisted Living Register requirements for social distancing, The Cedars will allow ONE designated visitor per unit. The goal of this registration program is to limit the number of people who may possibly bring COVID-19 into the building and into the building of your loved one.
To register as the designated visitor, you must complete the following form and receive confirmation of your application from the Cedars.
Once confirmed as the Designated Visitor, you will be able to book a time to enter the Cedars to visit. During your first visit, you will pick up a non-transferable ID badge that you will wear at all times while in the building. You will need to provide a photo for this ID badge.
Please note that bookings for Designated Visitors are not available yet. We are waiting for government approval of our safety plan before official visits can begin. Please check our News & Events page for updates.
